Submit Button is not working, just says "Please Wait"

We just found out that our application form has not been working for the last couple months.  When you click submit it simply says "Please Wait" and never submits the info and never sends the visitor to the thank you page.

We've tried cloning the form and the clone still has the same issue.

We've also tried deleting and replacing the submit button and still have the same issue.

I created a clone and tested the form. If you remove and add again the 3 spacer widgets of the form this situation will be solved:
