Unfortunately, I'm unable to understand your concern fully. I see that your account rmcguigan is already in "Free" account? But I do see that you have a similar account with a different username which is rmcguigan1, that one has an active subscription plan, would you like to cancel it?
If you would like to cancel your subscription, you can follow the steps below.
1. Log in to your www.jotform.com (make sure to log in to the account that has an active subscription plan).
2. Navigate to Settings->Billing->Choose the "Starter" or "Free" plan.
After cancellation, your account will remain in your current subscription until the expiration date of it, after that it will switch to "Free" or "Starter" plan and you will not be billed anymore.
