that was not the form i was talking about. when you come to the home page of UserReport.com the is a button that says "demo". when you click on that button the form shows up.
Yes, that would be possible, but you would need to add all the fields to your jotform and set it up to have everything you need and only then can we help you if you have any issues making it look just like it does there.

The reason for this is that all style might need to be altered with addition of a single field, so it is always best to style only once you have all fields on your jotform.
