I was instructed to upgrade my Jot form account or it would be disabled. I did upgrade the account at 9:07am here is a copy of the email I recieved.

Please let me know if this form is disabled or not as I have a major marketing campaign out that is driving ppl to this form and I do not want to miss out on any leads due to this mistake.

It seems that you've upgraded your account (teamchurgin) while you're logged out and hence the payment was not linked to your account. However, I have now manually upgraded your account to Economy and activated your account. You should be able to login into your account and see the upgraded status.
