As far as i can see, you created that form while still a 'Guest' user. i.e you havent created a full registered free user account.
If you now create a new user account, and register with your same @drake.edu email address,the forms created whilst a 'Guest' user should automatically get transferred over to your new account.
Try it, and let us know if you encounter any problems.
