In this example, though there are others, I would like to add one button to a google form that allows the user to upload a file. I have some experience with scripting, but not scripting inside google docs/drive. The files can live in dropbox or google drive. I just want to collect the name of the file in the google form/sheet and know that the actual file will be located in a shared directory on google drive or drop box. I feel so close, but can't get the right script in the right place. I've looked at a number of different options, but have not been able to implement. Please advise.

Adding a file upload field and integrating your form with Dropbox or Google drive is quite easy using JotForm. Just click on the file upload field in the form builder to add it to your form:

Neither requires and coding and takes at most a few clicks.

Is it possible to add just the upload button and related fields from jotform and have them show in the Google form? The Google form is already made, I just want to add an upload button. Is that possible?

It is unfortunately not possible to add JotForm fields to a Google form. They are two completely different platforms. You would need to recreate your form in JotForm in order to use our fields.
