Forms not being delivered to people on the email list

    Our company currently uses jot form for incident reporting. We have added emails of those who are to receive the reports and act on them.

    For some reason, (2) email addresses are not getting the emails. One was receiving them regularly until December and the other started in December & has never received them.

    We have tried other forms & the (2) employees receive them but not the forms we have created. A new employee was also added this past month and she receives the forms all the time.

    We have a link on our internal system that directs employees to fill them out. When the submit button is selected, an email comes into our system & delivers the email. We have discussed with our IT Department & they state that the email addresses are disappearing at your end.

    The (2) email addresses are jmounsef@streamflo.com & tthomas@streamflo.com.

    Please check as to why the Jot Form System is not sending the emails.

    The reports are:

    - Near Incident / Safety Observation

    - Damage Report

    - Occupational Injury Report

    I tried to download a word document with the screen shot showing their email addresses but your program only accepts jpg, jpeg, png or gif. I'm not sure how to change the document to one of these.
