I am making some changes to the form. The changes show up in the preview window, but not the website. I am copying the complete source code into my page, same as I have always done. I have refreshed the browser cache.

Yes, I can see the changes are not reflected in the embedded form:
Please try clearing your forms cache and then pasting the source code, if that does not work try embedding with the iFrame method. Let us know what happens, we will be glad to assist you.

I tried both of your suggestions. First I cleared the form cache. No luck, the website didn't change. Then I tried using the IFrame code, copying it into my page. Again, no changes to the website were seen.

Let's try one more thing, can you clone your form and try embedding that one? https://www.jotform.com/help/27-How-to-clone-an-existing-form-from-your-account
This is quite odd, I just cloned your form and embedded it, it looks the same as in the stand alone version: https://shots.jotform.com/david/InquiryForm.html
