My Multi-page form is getting stuck

My form is not allowing my users to click next.

I have tested your form on your website and I noticed that there is an error inside of your website.

The reason is that have embedded you form on page http://www.acesgolfclub.com.usrfiles.com/html/404bb8_41c87e3de8359293d5ea3fb9ff24c570.htmand form is working fine there, but you have embedded that page inside of your main page.
