Our Foster Application form is not working. We had a client submit an application and not only is it not showing up as a submission but was not emailed as it usually is. We need this fixed ASAP! Please respond soon!

The form is working fine the way how you embed it. You are currently using our oEmbed plugin and I see nothing wrong with it.
How many of your clients have reported this case to your form? Because it's impossible that you won't be able to see the submission in the form submissions page if they have successfully submitted the form and see your form's thank you message. There's a possibility that you won't get the form email, but the submission should still appear in the form submissions page.
Please ask the following to your client(s):
-If they saw the thank you message
-If they saw an error after clicking the submit button
-If they noticed that nothing happen when clicking the submit button

Just to add, you form email notification settings are correct and you are using Gmail address so it's impossible that you won't get an email when the client have successfully submitted the form.
