Drop-downs with allow multiple option revert to "0 items" on IOS

A mobile iOS user is experiencing difficulty with a few of the drop-down forms on my page. Whenever he chooses one of the options listed, the form automatically reverts to something called "0 items" (which is not part of the original form). He is not able to submit the form, which keeps telling him that these drop-downs require a valid choice.

It seems that the dropdowns with the "allow multiple" option on display "0 items" as default option on IOS:

But I tested and could fill in the form an successfully submit it:

Except for the fields "citizenship" and "How did you find out about this trip?" all the other fields can be regular dropdowns.

Please try embeding the form again, using the iFrame method to see if it solves.

Let us know if the problem persists.
