How can I made clients check a box to agree to a paragraph of terms?

    I'd like to make a list of terms for a client to agree to, where the client must click to check each paragraph.  This site has what I'm looking for:  http://www.bonnieirisphotography.com/newborn-session-contract

    I presume that you want to have the checkboxes for several different points as shown on that form?

    That is actually a jotform, so you should be able to just clone it to your account and alter the wording to your need.

    I was able to clone her form, but I'm wondering what features were used to create the check box near the paragraph, as I'd like to make a few more of them within my form?

    From what I can see, it was just the regular Checkbox from the Form Tools.

    It has only one option that has a lot of text(paragraph) on it.

    if you want, you can just dupicate/copy the field itself and just add/change to your own text/paragraph in the Checkbox Options value.
