    I see what you meant when I checked the form on the website.

    Please try first re-embedding the form http://www.jotformpro.com/form/50886358960974 in your website using its iframe embed code instead.

    user guide: -Getting-the-Form-iFrame-Code

    make sure to remove first any old existing embedded form script code before embedding again using the iframe.

    Hope this help. Let us know if this did not resolve the issue.

    Thanks.

    It was caused by responsive form styles. The following CSS has been added to your form in order to have the desktop like form layout.

    @media screen and (max-width: 480px), screen and (max-device-width: 768px) and (orientation: portrait), screen and (max-device-width: 415px) and (orientation: landscape) {

    .form-label {

      min-width: 150px;

      width: auto !important;

    }

    .form-input, input[type=text] {

      width: auto !important;

    }

    .form-radio-item, .form-checkbox-item {

      width: auto !important;

    }

    }

    Thanks!

    They should just offer that as the default ? So that the desktop looks ok, then it changes for responsive

    I assume all is good now.

    As far as I know, the default setting of the form is not set to have a responsive layout.

    It will become with a responsive layout if you enable it in the form during design.

    user guide: -How-to-make-forms-Responsive

    The default value of Responsive Form is NO. So, the form is not set to have responsive layout by default which will not make the fields to auto resize depending on the device browser being used.

    Perhaps Responsive Form was set to ON in your form, the reason the fields were auto adjusting on the browser viewer.

    Thanks.
