The phishing filter had automatically suspended your suspected form because it has fields that ask for account and password. This is considered a phishing form which is not allowed by JotForm.
You have to delete this form and try not create a similar form again before we can request re-activation of the account and other forms.

This form will be part of a member signin page for a website for those members. I don't think that's considered Phishing? Any way I can continue?

Unfortunately it is not possible to use password fields in Jotform, even with legitimate purposes.
Unfortunately this was an extreme measure that Jotform had to adopt to control phishing and it had good results.
The form was removed and the account is now active.
