Fields not displaying properly

    In trying to add a background color to my header (which I never ended up being able to do) - I ended up breaking something in the CSS code. Now any field that I add is not displaying properly. Here is the URL:

    I kept thinking that if I deleted the fields and re-added them it would fix itself - but no such luck. I just have the date field on page 1 at this point. Fortunately - the rest of the form did not get messed up because it took me many hours to get that format in place.

    Hello armskite,

    Do you mean to say that the width of your form fields are displayed in the full width? Is that the issue you are having?

    There are several custom css code injected in your form which seems to be causing this issue. When I check your form with its direct URL, it seems to display correctly. Please check the screenshot below:

    The easiest way to solve this issue is to revert back your form to an earlier version. You can take advantage of your "Revision History" feature. The following guide should help you:  https://www.jotform.com/blog/95-Introducing-Form-Revisions-Never-Lose-Your-Work-Again-

    Thank you so much! I was panicking!

    Two other questions:

    1) How do I change the background header color of my two other forms to match that of my client intake form (i.e., the purplish color) - that's what I was attempting to do when I messed up the sizing of the form fields.
     

    2) Just to clarify - I have a form called: Weekly Feedback. I will have the each client filling out this form weekly - each week when a client links to the survey will the fields be 'clear' so that the form is once again ready to be completed? If not, how do I make this happen.
