    Why does the contact form come out blank ?

    I have created a simple order form but sadly all the fields come out blan when they are submitted to me.

    Any ideas why ?

    Hi Anthony,

    I have checked your link and wasn't able to find JotForm's Form, is that your manually coded form? The form you had right now isn't from JotForm, I have also checked your account and found nothing. If you would be so kind to share your JotForm's FORM URL then that would be a great help.     I am not sure if I have done this right but I designed the form myself.

    Jotform acc name is certificates

    Hello,

    If you would like to customized your JotForm's FORM in your own style you have to download it's full source code to ensure that e-mail submissions will be processed via JotForm unlike the form you are currently using which is processed in your custom code (sendmail.php). You can also use our embed features for posting your form directly in your website.
