I manually checked your forms and they are quite obviously being used for phishing.  There is no other reason to collect the usernames, passwords and security questions for facebook accounts.  All of which are strictly against our terms of use:

The account and forms are both suspended.  For something very obviously trying to take over Facebook accounts, it is hard to reactivate the account.  If you can provide a good reason as to why you needed to collect that information, we can reactivate your account.  Other than that, it will remain suspended.
