    I am trying to get the monthly activity report template going. I am lost. Do you have any simple instructions to use

    I cant seem to enable default fields to put info

    I'm not sure if I understand your query correctly. Could you provide us with more information on the issue that you are experiencing with your JotForm?

    If you are referring to the Form titled "Weekly Activity Report by Rohan Welangoda", I see that the account (rohanwelangoda) associated with this JotForm is suspended. It seems that there is a minor issue with the account.

    Let me forward the issue to our next level support team to take a look. You'll be posted through the thread once we have any update on the issue.
