Greetings,
You can create a tabbed form using the widgets provided by jotform.
You will first need to add page breaks to the form so that you can use the tabs widget, each page break defines a new page for each tab, under survey tools in the form builder select page break and add it to you form.
After adding the required page breaks, add the tabs widget next, it is accessible under the Widgets tab: Widgets->More widgets.
A widget widow will open, in the drop-down select survey (this is where the tabs widget resides) to filter the widgets list.
Scroll down to the options presented and select the Forms Tabs widget
Upon selection a new window with the widgets properties will be opened, by default titles will be created with names like Page 1 Page 2 etc. if not titles are defined the tab heading will default to numbers 1, 2 e.tc according to the page breaks added
Change these names to match the number of page breaks you have, each new line represents a tab and the tab title. You can also specify additional options such as custom css and form header positioning.
After you've defined the tab titles and number click on the finish button to save the changes.
Click on the preview button to preview the form with the tabbing applied.
