How can I place a table within a form, it needs to include a check box in the first column and then a quantity field in the second?

    How can I place a table within a form, it needs to include a check box in the first column and then a quantity field in the second? 

    An answer ASAP would be much appreciated.

    Are you trying to make an order form? If so, the quantity field can be added to the products list from the payment wizard.

    Please refer to the the guide How-to-add-options-to-Payment-Forms that can help you in adding options in the payment wizard.

    If you are referring to have only checkbox besides a number field, you may need to arrange the fields using field positioning like below by having separated checkboxes and input box fields :
