Form doesn't come right on iPAD or Android devices

When I load the form from Note 4 or iPAD mini the same doesn't come entirely. The submit button is missing and isn't as responsive as it shows on the design page. 

I have tested your form outside of your webpage and it is responsive.

I also checked your website and I noticed that you are using script block (source code) for embedding your form on website. We recommend using of iFrame code. You can try to replace source code with iFrame to see if that will display your form correctly. In this article you can find out how to do it.
