How can I add a word or pdf File download URL in my form?

    I have a summer camp registration form and I want parents to be able to access a pdf document on the form that they can download/print/save for their records.  Basically, it's an information sheet for them to keep that has details of the summer camp (times, dates, locations, etc.) that they could print and hang on their refrigerator.  It would be a completely separate document than the registration form and would only be accessed by the parents if they request it (i.e. click here to download the pdf document).  This would be different than an auto-responder email that they would receive once they submit their registration.

    Is it possible to do this?

    and would only be accessed by the parents if they request it (i.e. click here to download the pdf document)

    It seems your do not want to embed the PDF document in your form but just add a download URL so that users can click on this URL and download the PDF file. Is that correct?

    You just need to upload the PDF document in your own server and add the URL in the "Text" field of your form. Please check the screenshot below on how to do this:
