I'm sorry for the trouble caused to you.
I did test your last edited form and I am able to replicate the issue you are having. The issue seems to have caused by the google drive integration you have in your form. I would suggest you to remove the google drive integration from your form and integrate it again. That should hopefully solve your problem.
