No Confirmation Screen

Hello, I am using a jotform on www.softballforvets.com and i am not getting the jot form to automatically forward users to the seconary form.  Do I have a setting wrong?

After trying different settings, I believe I had a setting wrong.  In Preferences->Advanced Settings->Send Post Data, I had set it to 'NO', when, it needed to be 'YES'.
