How do we reset the user name and password for a previous employee at Holy Spirit?

    Matthew Spiess was a previous employee of Holy Spirit Church and had set up an account for several JotForm applications.  He has left and the only information we have is the user name (HolySpirit) and password (rosary97) but these do not work. His previous email was mspiess@holyspiritsa.org and this email address is no longer active.

    How do we get the account under his name to my name and email address?

    The account's email address is: danchavarria@holyspiritsa.org, if you have access to this email address you can click on "Forgot password?" when attempting to log in at www.jotform.com, sp the reset instructions will be sent to that email address.

    If you do not have access to that email address, could you provide us the last four digits of you credit card number which is used to pay the subscription?
