    Hi, if what you meant was to have an image launch your form, it can be easily be done by using the built-in Embed Form feature.

    Please go to 'Setup & Embed' > 'Embed Form'.  There are two options that can quickly do the task that you desire, namely the 'Lightbox' and 'Popup' methods.

    It will give you a code like below:

    <a target="_blank" href="javascript:void( window.open('http://www.jotform.com/form/12953920476', 'blank','scrollbars=yes,toolbar=no,width=700,height=500'))">Contact Us Sample</a>

    Simply replace 'Contact Us Sample' text with an image tag for HTML, e.g. <img src="//max.jotfor.ms/images/logo.png?v3&v=3.0.1708"  />

    ---

    So you will have something like this to be added in your HTML page if you used the 'Popup' method.

    <a target="_blank" href="javascript:void( window.open('http://www.jotform.com/form/12953920476', 'blank','scrollbars=yes,toolbar=no,width=700,height=500'))"><img src="//max.jotfor.ms/images/logo.png?v3&v=3.0.1708"  /></a>

    or like below if 'Lightbox':

    <script src="//www.jotform.com/min/g=feedback" type="text/javascript">

    new JotformFeedback({

    formId:'12953920476',

    base:'http://www.jotform.com/',

    windowTitle:'Contact Us Sample',

    background:'#FFA500',

    fontColor:'#FFFFFF',

    type:false,

    height:500,

    width:700

    });

    </script>

    <a class="lightbox-12953920476" style="cursor:pointer;color:blue;text-decoration:underline;"><img src="//max.jotfor.ms/images/logo.png?v3&v=3.0.1708"  /></a>

    Please let me know if I misunderstood your question.
