My free form quota was full, but I paid for this month's service. Form is now gone in Jotform dashboard.

    I'm able to verify that your account is labeled Premium. Do you have some reference to the form that is missing other than the page url listed above?

    Maybe you have an ID?

    I can see 2 forms associated with your account at the moment.
