    Hello,

I built an online application form for a school I work at. One of the required functions is to upload a photo of yourself. Initially, this worked every time with no problems. Now, the forms come through with missing photos about half of the time. I then need to email the applicants and request that they send me photos separately. It does not look great, so I would love to fix it. Please let me know if there is anything I can do about this.

Is this form embedded in a sort of webpage? I can see the email notification seems to be fine. But you are right the photos are not included. Please share the URL to the page if this is the case.

    Good morning,

No, the form is not embedded anywhere. I simply link people to it and have it open in its own tab. Here is the one of the URLs of the page the link sits on:http://misdami.org/montessori-teacher-training-california/admissions-montessori-training-schools/apply-now/

That said, the link is on many of our pages. It is not embedded anywhere at all. Let me know what else you need. Thanks.

    Alex

Nothing came through at all. I looked in my form submissions on JotForm, as well as in my email where all the notifications are sent. So far, nothing has been received.

Could you please try to remove the "Upload a Passport-Style Photo" field on your jotform Alex and then try to submit the form and see if you get it.

Now to have the data that was previously uploaded to your jotform shown to you again, you should add it back, but use the same name - same widget and same name.

That should allow you to again see the data that was uploaded with the old field.

The reason why I am asking this is because there seems to be an error with it that happens after you click to go to new page.

If you do not want to do this, you can also clone the form and try to submit it and if the issue is still present, to remove the same field on the clone instead and see if the steps above help to resolve it.

You can see here how to clone your own forms: How to clone an existing form from your account?
