Survey form submit not working

We have a survey form that has been working fine for a long time. Today we noticed that the form goes to a blank screen once someone submits it. The submissions are recorded. It should go to this page: http://www.decksdirect.com/freight-shipping-agreement/thank-you

I have tried to embed the for, use and iframe, and source code with no luck.

I also tried deleting the forms email notification but that didn't work.

Please help.

BUT our dev was able to fixed and resolved them immediately.
