When I try to login, it either shows a blank screen or it says that my login is incorrect.

and it is currently Active and in good standing.
In case you have forgotten the login password, you can reset it using the 'Forgot Password' link in the login page http://www.jotform.com/login
The reset instruction will be sent to your account email i###@###salem.org
