I'm trying to view the form i created recently however it comes up with the following error message. Please find attached and please help asap.

I took a look at your account and I see that it is autosuspended because of the "Deposit" form.
The reason are username and pin fields.
Please remove the same from your jotform and your account should be enabled once again.
If not, do let us know and we would be happy to check what is the cause and help you resolve it.

I see that you have opened 2 more threads in regards to the same issue. Please reply to this one directly instead in order for us to assist you properly.
If you follow the instructions above however, you will be able to reactivate your account with few simple clicks.
