I have a jotform that linked to a google spreadsheet. When the form is complete all information is captured except to name, address, City and State. how do I fix this?

Please redo the integration process also.
Follow this steps.
1. Remove the existing Google Spreadsheet integration
2. Save the form
3. Redo the integration.

4. Save the form.
Check the Google spreadsheet in your google account and see if all fields/data from the form submission are now also in Google spreadsheet.
