Can I customize the appearance of the submissions PDF file?

We are using an employment application in our lobby for new applicants to complete electronically.  Upon submission the form goes to my account at the JotForm site.  I can convert the form to pdf, but the format is different than our original form.  I would like to download and print some of the completed applications, but in the original format.  Is this possible?  You could call me at 574-658-5340.

We will have about 50 or more applications completed per month.  Should we upgrade to the next paid level?

Unfortunately, it's not possible to customize the appearance of the submissions pdf file. If you want to print the submission with the same appearance of your form you would need to use the edit link in your notifier so you can view the form with the entered data and print it.

Free accounts are allowed with 100 submissions per month, so you should be good to go with the free plan. If for a reason you need to increase that limit then you'll need to upgrade your account.
