I have a client who cannot fill out the forms on her smart phone. I am able to on my iPhone.

    Hello mb1940,

    I did check your web page where you have embedded your form and found that you have used your form's script embed code.

    Please be noted that for forms to work correctly in mobile devices, we recommend to embed form using its iFrame embed code. Please embed your form using its iFrame embed code and that should solve your problem. The following guide should help you:  http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
