Why are features disappearing from my jot form?

I created a jot form for my school.  I integrated it with google sheets and it was working perfect now about a week ago.  Now I went to use it again and the integration is no longer attached to the form so when i went in to get the information its not on the google sheet like it should be.

Apologies for this problem. We had some serious issues about spreadsheets, just until today. It should work as expected after re-integration.
