Form data not submitting to mailchimp

hi,

I have integrated mailchimp list but the form data is not getting submitted to the mailchimp email list. 

Kindly let me know how to fix this issue.

You are trying to integrate upload fields and they are not supported by Mailchimp:

Also please use one text box field for the full name field instead the complex "full name" type.

After making these changes please remove and integrate again with Mailchimp.
