I checked your form on mobile and I was able to view it without issue. If you had sent your client the link above, it will indeed not work. This is the proper link to your form:
The /jsform/ part of your link is intended for embedding the form.
If you have your form embedded to a webpage and that is what is not loading properly on mobile, let us know the page and we will be happy to have a look.

I checked the form embedded to your webpage on Android and indeed, I am unable to see it. I would recommend trying to iFrame embed code instead of you current embed code:
Replace your current embed code with the iFrame code and see if that allows for the form to show up on mobile, if it still does not, let us know and we will be happy to have another look.
