I have done everything previously stated, and I am still not receiving email notifications. My email is not on the bounce back list and there is nothing in my spam folder. I also tried to set up a sender email and that is also not working. Can you please help?

If you will check your form email history, you will see that form emails were successfully delivered to your email address.
Your configurations are correct so the problem could be related to your hosting provider. You have to add us to your server whitelist if we are blocked there. We have listed all our IP and Domains here:
Now, if whitelisting won't work for some reason, you have the option to configure an SMTP details to your account and use it to your forms. We have two methods:
a. Get and SMTP details from your hosting provider: http://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form.
b. Use Mandrill's SMTP: http://www.jotform.com/help/236-How-to-use-Mandrill-to-Send-Emails-From-Your-Own-Email-Address.
We recommend Mandrill if your hosting provider limits outgoing emails for using SMTP. Mandrill allows you to send 12k emails per month.
