Is it possible that your site is being blocked by Bellsouth, our ISP ? I can't access my forms from my home connection with Bellsouth DSL, but I can from my office with a fiber connection. And my neighbor nextdoor can access the form with his Comncast cable connection. I tried IE and Firefox The form in question is at

One of our servers had an hard disk failure. Since it wasn't down completely and only giving errors occasionally, we did not catch this one immediately. The problem is now fully resolved.
JotForm should now work completely since other servers have taken over the load. You should not see any further errors.
