Form Conditional Statement not working due to existing scripts in the TEXT field

    I set up condition on form below so that when the field titled "Ever been diagnosed with or treated for any of the following?" with boxes for heart disease, diabetes, cancer, etc is filled, only then should the box below it titled "Please provide details to above" show up.  But it's not working correctly.  I suspect I could have done something wrong, but I've set up conditions like this in past and they've worked.  

    Can you please help me.

    Please apply the following modification/update on the form.

    In the TEXT/Header on top, there were html scripts that was causing conflict with the form's scripts that prevents the conditional statement script from working correctly.
