Form Conditional Statement not working due to existing scripts in the TEXT fieldAsked by gconwell3 on April 11, 2015 at 02:48 PM
I set up condition on form below so that when the field titled "Ever been diagnosed with or treated for any of the following?" with boxes for heart disease, diabetes, cancer, etc is filled, only then should the box below it titled "Please provide details to above" show up. But it's not working correctly. I suspect I could have done something wrong, but I've set up conditions like this in past and they've worked.
Can you please help me.
I was able to figure out what was causing the Conditional Statement failure on your jotform http://www.jotformpro.com/form/50795017422958
Please apply the following modification/update on the form.
In the TEXT/Header on top, there were html scripts that was causing conflict with the form's scripts that prevents the conditional statement script from working correctly.
I have apply this corrections on my test form http://www.jotformpro.com/form/51007326687961 and it makes the condition logic work properly
Hope this help. Let us know if issue persist.
