I created a form and now I can't find it. It still works on my web site. Where is it?

When I log into my JotForm account and go to "my forms" the form I created is no longer there.  But when I go to the web site where the form is used it shows up and is working correctly.  How do I go about finding this form on my JotForm account?  If I need to make changes to it I want to be able to find it.
