Hey Guys!
I'd like to make this form:

1. The image at the top "payroll report" with the logo - does not show up when I print the page. On the other form I am comparing it to ("daily report") the image and logo at the top print fine.
2. I would like to decrease the margin space between the headings "Employees, Trainers, Class Instructors" and the configurable list table below each one to reduce waste white space.3. I would like the fields in the configurable list to be the same style fields as the ones in the tables here:
http://form.jotformpro.com/form/510159651549584. I would like to remove the "Bold" font from the column headers on all of the configurable lists.5. I would like to reduce the width of the fields in the configurable list, by 10%6. Additional changes as shown on attachement.

Hi,
It seems there are a lot of conflicting CSS code and styling that you have in your form, is this a cloned form from the original one? I cloned your form and edited it, here's mine: http://form.jotformpro.com/form/51021466352951. You can see that the logo is now displayed in the Print preview. You can start using my form by cloning it and use it instead, here's a guide: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL. After cloning, you can check the CSS tab under the Designer Tool, you can check the custom CSS code that I used.
