Hey Guys!
The form cover on this form, is not showing up when I attempt to use the "print form button"

I see the form cover is not taken when printing the form, I will report this to our second level, meanwhile, I would suggest you to remove the form cover:
And use the Image field:
It will be shown when printing:

Thank you, I knew how to do that option, but the form cover lines at the top the way I want the form to look. The image you inserted does not match the same size and alignment.
Let me know when they have the bug fixed, thank you!

It is important for me to have the form cover line up this for consistency, as I will be using this layout and format for other forms in my business. Thank you!

Hi,
It seems that you have a lot of duplicate threads and it would be hard for us to give you a final working form. I have replied to this thread regarding the form cover not being printed: http://www.jotform.com/answers/551024. The problem with the form is that it has incorrect positioning of the fields that pushes the form cover out of the original form area. I've edited the form and cloned it, you can check the other thread for the changes that I made.
