For the last ten or so submissions, the first child's name is not filling in (or filling in where the second child's name should be) and I am not getting parent's address information in the data spreadsheet. Can this be fixed? I am not sure why it stopped working.

We were indeed having some issues earlier with excel and CSV downloads of your submissions. This has recently been fixed. Try downloading your submissions again to see if the output is back to normal. If it is not, let us know and we will be happy to have another look.
