Also, in trying to fix this, I have thrown off two dropdown fields that need to be properly centered in the form:
    #input_19, and #input_22

I'm checking your form http://www.jotformpro.com/form/51015965154958, and it seems that there are a lot of problems on the positioning and aligning of the form fields. Could you share us what needs to be properly aligned and how it should look like, I've cloned your form and can make adjustments in my end, you can then just clone mine to see the changes.

There also seem to be problems on the custom CSS codes that you are using. Here's my cloned form: http://form.jotformpro.com/form/51028113743951?. See if this one works, you can clone it using this guide: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL.

Do let us know if you need more adjustments on this.

    Kind regards.
