Issue with image upload

    I keep getting this error message on my form while using it on my mobile.. I have a form that needs the user to send 5 images. here is the message (a problem occurred with this webpage so it was reloaded) keeps coming up when i use my mobile to upload second image.

    Are you referring to the form embedded on this link: http://www.threadswap.ie/#!list-items/c1at6? The form there is this one: http://www.jotformeu.com/form/51001474647349. But I can't see the upload field there for the image? Am I looking on a different form and different web page link?
