I work for Raycom Media and I have a station that has sold an idea to a local church and they loved it, but now we are trying to figure out how to make it work.
The idea is called "I-Act", which is kind of like the "iContribute" and others. The local church would like for community members to nominate certain people they know that has done great things and to it upon themselves to act on a good deed, etc. They will then read through the nominations on a monthly basis and choose a winner to receive a prize of some kind.
We simply need a tool that can be implemented into their website (www.fox54soconnected.com) on a landing page we create, so that the community can be directed there and can fill out a simple form nominating a particular person. The information that form collects would need to be turned over to the church monthly.
Is this something your service can provide? If so, how do we get started and what are some of the things we would need to be aware of from the cost down to the creation steps?

If someone needs to define the name they nominate, you can add a Text Box field in your form. This field is in the "Form Tools" section in our form builder. If it's a predefined list of names, you can use our Radio Button (for single selection) or Checkbox (For multi-selection), these fields are still in the Form Tools section.
For Data reporting, you can either create a report and submit it to the church. You can find our reporting options from this section:
I would suggest exploring our form builder and create your form. You can also check for templates from our form template gallery: http://www.jotform.com/form-templates/ and use a form.
As for the cost, you can register for Free and use our entire features for free. We only have a limit in terms of a monthly submission and file storage limit. For example, free accounts are limited to 100 non-secure submissions per month. If you need more that this, you can upgrade your account to our Premium Plan which will allow you to receive up to 1000 submissions per month.
