    Hi there;

              Please try to clone this form (http://form.jotformpro.com/form/51035263526954) or inject the following CSS Code Block at the end and use our "Mobile Responsive Widget". Review the following screenshot it is working. Read our guide here on How to Inject Custom CSS Codes.

    @media only screen and (min-width: 640px) {#cid_12 {margin-left: -50px !important;} .form-multiple-column, .form-single-column {margin-left: -20px;} #id_2, #id_13 {margin-left: -40px;}}

    Test and let us know, if you need further assistance, we would be happy to assist you.

    Thank you! This worked to center the top image, but the submit button and "terms and conditions" image at the bottom are now a bit left of center. Can you please send css to adjust these as well?

    I was able to see the difference between your form http://www.jotformpro.com/form/50995633643969

    and my colleague's cloned version http://form.jotformpro.com/form/51035263526954

    Please take note that Syed had also added the Mobile Responsive widget to the cloned form.

    that could be the part that is making the difference.

    How about if you just clone the corrected form instead  http://form.jotformpro.com/form/51035263526954

    User guide: -How-to-Clone-an-Existing-Form-from-a-URL

    then just make all your adjustments on the cloned version of the form.

    Hope this help. Let us know if you need further assistance.
