Thank you for your interest in using JotForm. Though creating quote forms or order forms is easy using JotForm, the form that you have provided seems to be very customized and using our inbuilt payment tools may not give you the same design. Also, I see different calculations are existing in the form provided and it requires a custom designed JotForm and the final calculated value can be sent to the payment tool.

You can use our Form Builder to add the fields to your JotForm by using drag and drop method. You can use the calculation widget to perform calculation based on the field values set on dropdown fields. The fields can be hidden or shown by using the conditions wizard.

The form can also be customized to bring a great look by using the Form Designer and adding one of the hundreds of themes available at our theme-store.

Hope this information helps you creating your online quote. Please do not hesitate to get back to us if you need any further assistance in this regard. We will be happy to help.

    Thank you!
