Can't make it work with WP

Form works well within your site but the integration to WordPress doesnt work for me. i'm using Salient Theme

here is the code i'm pasting in a textbox :

I also tried to install the plugin but result is the same :

I was not able to view your WP website because the URL you had provided was from your WP admin panel

but checking further, I was able to find it on this URL http://ascod.ae/?p=4

I can see that the embed code is not working properly on the WP page

I suggest to please retry from start and follow the user guide 

-Adding-Forms-to-Wordpress-org-version

Make sure to clear first all previous existing embed code before re-embedding again.

Embed the form code when adding/editing the WP page in the TEXT/HTML page
